Your First Visit

When you arrive, our front desk staff will greet you and welcome you to our clinic. If you have not already filled out and submitted your new patient forms, we will request that you do so before seeing the doctor.

Next, you will have a consultation with one of the doctors to discuss your health-related problems and concerns, as well as potential treatment options. Your first visit is designed for the doctors to learn more about you, your condition and expectations, and to determine whether chiropractic care meets your needs.

Payment Options


We accept some health insurance plans and automobile insurance plans (if you were involved in an automobile collision). Since there is a large variety of plans and coverage widely varies, please call us to find out if your plan covers care in our office. Our staff will verify and explain your benefits at no charge and before treatment begins.

No Insurance

For patients who have little or no insurance coverage, flexible payment programs can be arranged. If you have no insurance there is still a way for you to receive the care you need. Many patients pay directly for care, as they discover that care in our office is extremely cost-effective and affordable.

New Patient Health History Form

This form lets us know the history and current state of your health. Do not fill out these forms if you were in a car accident, fill out the forms below.



Auto Accident Form

This form is to be filled out and printed prior to your visit ONLY if you were involved in an automobile accident and that is the reason for your visit. If you are filling out these forms you do not need to fill out the "New Patient Health History" Forms.
